You, unfortunately, do not say what OS you are on, but on Solaris this is in
/etc/LGTOuscsi, on Windows it is in a subdirectory of the nsr directory

Hmmmm, my next step would be to validate the hardware paths
match up to the device paths as expected.  For example, if you
load a tape in a drive then try a tape drive operation outside
NetWorker, does the operation run against the loaded drive or
does it also produce an error?  I'm not too familiar with Windows
server set-up, but the error seems to indicate that the target
device is offline or inaccessible, a condition that could arise
from a change in assigned device names or SCSI IDs.
