Koen,
There were a couple of "media emergency: cannot fetch volume" fetch type
errors in 6.1.1, LGTpa33799 and 35855 are two that I am aware of. Don't
know if the exactly fit your symptoms.  I believe that support can supply
you with hot fix for both, but upgrade to 6.1.2 is the pernament fix for
them.
