You might wish to check your controller for any device errors.  This usually 
refers to either a bad drive or bad SCSI controller, if you're running SCSI...

What's your system config?  You never mentioned that part of the equation...

option) would produce

nsrjb: SYSTEM error: I/O error

Issue nsrjb command again and you get the right output.
