I agree with Prashant. it looks like a device driver issue. try using the
TSM driver for the medium changer and Compaq drivers for the actual drives.
and be sure to turn off Window's RSM.

This seems to be the issue with the device driver conflict, between the TSM
device driver and win2k device driver, try disabling the windows tape
device driver and use the TSM device driver.
