It is the DT Serial option. You need to find it in the configuration menus
(I think it's in config, library) and turn it off. Then it will work. This
is a new problem. The Qualstar microcode was updated to use this SCSI
option differently which exposed a bug in TSM (I believe it's a bug). If
you turn off DT Serial the library will define just fine.
