I have two FC64-1063 cards in an E4500, connecting to an EMC array.  I
installed the driver supplied by EMC (fcaw259).  At this moment I'm not
actually sure whether it's connected direct or through a switch - sounds
nuts, I know, but the server isn't in production yet, and the SAN stuff is
being arranged by other people.
