I/O Errors

Dear ADSM Gurus:

We get the following I/O errors intermittently and when each time the errors
occurred, the DLT media volumes that was mounted last, the access mode is
changed to "read-only" or "unavaible" due to write error.  (Samples below)
Have anyone encounterred this before?
