I've seen quite a few of these recently - IBM 3466, 6 x 3590E drives, ADSM
3.1.2.42, AIX 4.3.3. IBM analyzed the logs and recommended changing the
head-cleaning brushes on the drives, plus yet another drive microcode
upgrade - this time to level D01D-22E. Not had any problems for a few days
now.
