        fortunately I didn't find this problem (yet!).

But ... thinking:

the references to that cassette are from the DB.
the reclamation process on cassettes in the VAULT works on
  ONLINE cassettes.

hence the problem is somewhere in, either the DB or the cassettes in the
primary STGPOOLS?
