We had a lot of problems with our B-Drives and B-Tapes...

To make a long story short: we worked on several problem areas in parallel and
right now (knock on wood) we do not have any problems with our tapes. This is
what we did:

have an IBM CE replace part after part of the robotics, drive heads, drive
electronics etc.
have IBM "media service" replace approximately 20% of our tapes
change the SCSI card (right here it was an Adaptec in a IBM PC NT box)
update the SCSI driver
update the SCSI driver
update ADSM Server
monitor tapes carefully (we randomly do "audit vol" on all of our tapes)
we also added all volumes to the storage pool and don't use scratch tapes any
longer
