There is an open apar IC23176 for this problem.  The problem is with
SMDR.NLM.  We regressed the SMDR.NLM back to the base NetWare 5.0, and we
have not experienced the problem again.  I took a quick look at the APAR and
here's part of the contents...

    This problem is due to a problem in Novell's
    SMDR.NLM module. The SMDR.NLM 5.03 level of the
    module fixes the problem.
