Q1 Yes a RESTORE VOLUME would have been enough.

Q2 Delete the volume from your library and prevent ADSM from reusing it.
    Do an OFFLINE Analysis using a tape analysis tool. if tape is ok put
it back to your
    library if not throw it away.
