The volume may be in the wrong slot.  Is ADSM acting as the 3995 controller
(Directly Attached to the AIX box)?  If so, run a 'AUDIT LIBRARY libraryname".
It will mount and check the label of each cartridge, and rebuild the table of
which slot holds each cartridge.

On a C68, the audit takes around one hour (258 slots, all filled).
