Error defining device

Attention ADSM support and all the ships at sea!
Anyone know what may be the problem?

We just installed ADSM on AIX v4.
We defined a lago data wheel library to ADSM.
When we try to define the tape drives we get the following error:

    ANR8420E DEFINE DRIVE: An I/O error occurred while accessing drive TAPE1.

  Here is the infomation on the LS-380L problem.

  (Library SCSI ID = 2)
  Peripherial Qualifier = 0
  Peripherial Device Type = 8
  Vendor ID = LAGO SYS
  Product ID = LS-380L
  Revision = 4.9

  (Tape Drive SCSI ID = 3)
  Peripherial Qualifier = 0
  Peripherial Device Type = 1
  Vendor ID = EXABYTE
  Product ID = EXB-8500-85Qanx0
  Revision = 047Z

  (Tape Drive SCSI ID = 4)
  Peripherial Qualifier = 0
  Peripherial Device Type = 1
  Vendor ID = EXABYTE
  Product ID = EXB-8500-85Qanx0
  Revision = 047Z

  I then issue the following command:

    DEFINE DRIVE LAGOLS-380L TAPE1 DEVICE=/dev/mt0 ELEMENT=54

  I then get the following error:

    ANR8420E DEFINE DRIVE: An I/O error occurred while accessing drive TAPE1.
