Hi Tom,

I was having the same issues over here, we have basically the same backup 
platform as yours. Two SL3000 with five IBM LTO4 drives in each one running on 
NetBackup 6.5.4, this problem was driving me nuts, I've done a lot of firmware 
upgrades until I fixed this a couple of weeks ago.

First you gonna need this:

Update your drives with this firmware, using IBM Tape Diagnostic Tool (ITDT), 
you can find it on:

Then open a support ticket with SUN, to update your SL3000 firmware to at least 
2.35 release. remembering that firmware updates are user applicable only on the 
drives.
