Do you know exactly what I would look for, what the permission change
is?  I am not a windows guy so any help would be appreciated...

I've seen this on windows file servers when one of the admins makes a
change to permissions at the base of a directory tree. The permissions
change sets the file as needing a backup. It's really annoying when
they make a change at the root of the filesystem and I end up doing a
300G incremental.
