We have found the same thing. We have noticed also that the media databases, if 
separate from the master server, will retain data from failed jobs on tapes. We 
routinely check for this and deassign and expire tapes each week.

I have no confirmation of this but I've always had the sneaking suspicion
that the fragments of a failed job still occupy the tape, but are unusable,
until that tape expires.

One option you have is to use bpduplicate to copy the good image to a clean
tape and then expire the original tape.

Ok. It looks like I can't expire the job because it's just not there.
However, 
there's this report generated by the available_media script that still
claims 
that the tape is full (if that job wasn't there anymore it wouldn't be
full).
