[Veritas-bu] Media Database Inconsistencies

I'm new to this list and new to using NBU, so forgive me if this is a dumb
question.  

In viewing the Reports | MediaList, many media that should be on the list
are missing.  They can be viewed in the VolMgr database via VMQUERY -m <ID>.
The VolMgr data seems to be correct.  Medialist shows some 250 media and
there should be more like 450.  Medialist doesn't even include all the media
that are presently in the library.

Why does this happen, and how can I correct the inconsistencies & prevent
recurrence?

System info:  
NBU DataCenter 4.5 FP6.  
Win2k Adv. Svr. SP4  
Dell PowerVault 136T.  
1/2" HCART 100/200GB.
