We had the same problem a while ago and the storagetek guys came over a few
times before resolving it. The problem was resolved by updating the drives
firmware and manually testing every cartridge. Some of the tapes were
corrupted and a code appeared on the drive when they were loaded. Sometimes
those tapes would get stuck so we would see the down drive in the morning
and we would take them out of the library but some of them didn't actually
end with an error the last time they were used so the next time they would
get loaded in the drive we would get the error you have. My point is that
the fimware upgrade resolved the problem but that some "tricky" tape would
get us some error again the day they got mounted so each ones had to be
tested. The error you have is the fault of the tape but because it was
corrupted by a wrong header. We were actually able to reuse those tape after
formatting them (and losing all datas) to data tapes from the drive.
