I've seen this problem before in my environment.  The issue was that a
large library in a SAN environment had been rebooted and upon coming
back onto the fabric the host (with multiple tape drives) rebuilt the
device aliases automatically and appended those to what it already had.
So on my host (a netapp filer) which normally had st0 pointing to the
tape drive 1 and st1 pointing to tape drive 2,  now actually had st2
pointing to drive 1 and st3 pointing to drive 2 (leaving st0 and st1
stale).  Vertias was configured to talk to st0 (nrst0a) / st1 (nrst1a)
and after putting a tape into what it thought was the right drive (st3
which was actually st0 for a tape drive on another host) it couldn't
read the tape because the tape was physically in another drive.  Veritas
would eventually time out trying to read the header on that tape and try
another tape over and over again.
 
<reading over what I just typed I realize this sounds kinda complicated.
but it's really not>
 
The quick fix was to delete the aliases in my host and rebuild them.
The long term solution was to create a 'alias-device' file on my host
(for the life of me I can't remember the name of this file) that
persistently binded the correct devices to the correct alias even after
reboots.
