I was having this problem with my Unix media server.  I chased the problem,
with Support's help, to a bad driver setup that was using fixed-length
blocks instead of variable length (st.conf file in Solaris).  Once corrected
the problem went away but the previously written tapes are more difficult to
access.  

I realize you've got a NT server but this may be a starting point.
