I had same SCSI I/O error, and i looked into every level of my system and 
found out that the firmware on HBA card attached to the server was the 
culprit.

Reason was that my emulex HBA cards was running on updated firmware 3.9 and 
my tape drives were not able to digest it, so i downgraded the firmware to 
3.0.3, and its working perfect now.

According to the "SCSI transport failed: " error that Craig listed, it is
nothing related to the tape. the reason is the device doesn't response or
delay to response the request from the host. I would suspect some hardware
issue between the ADIC and the host.
