When I had that problem, it turned out to be due to the line in
st.conf (on our Solaris master server/media server) for our 9840 tape
drives. Veritas tech support found this problem and told us how to correct
it. I sent an earlier post to this list describing what the fix was.

I've got in the last week error 86 (media position error) for some jobs.
The dlt has remain in drive and the drive went down.
Have anyone an explanation, what is going wrong if this error occurs?
Is it maybe a timeout problem (it took a long time to rewind the tape). Is
there a parameter to change a timeout for positioning tapes?
Environment: NBU 3.1 on HPUX 10.20, DLT Library HP 4853A
