Josh,

If you can determine the media-id that is causing the problem
try running "vmquery -deassignbyid media_id pool_number status"
You can get pool_number and status by doing a "vmquery -m media_id"
I have seen this problem before where for some reason a media
expires and is removed from the media database but does not get
deassigned from the volume database.  Hope this helps.
