We opened a case with EMC, and this is apparently a known issue with NMO
4.2. To address it, we needed to add NSR_MMDB_RETRY_TIME=30 via parms.

It's mentioned on page 33 of the NMO 4.2 release notes, but not in the admin
guide, unfortunately.

We applied the update a week ago, and haven't had a problem since :-)
