Hello,
Have you implemented persistent binding ? or  software tape driver 
capability to have persistent tape  name, check IBM Lto drivers release 
notes on how to do this.( i believe now windows 2003 and above  has a 
registry to achieve the same)
Also, try nsrjb  -HEvvv might clear up the condition if reboot again does 
not fix it, beats recreating the tape lib.
Occasionally, the tape may get stuck in the drive and then only a service 
call can take care of it.

I've seen this occur when the drive mapping has been lost... Did you
recently reboot the server ?

Every time this happens to me, I have to delete and re-create my jukebox
instance....

It puts a tape into what it thinks is \\.\Tape0, but it has really put
the tape into \\.\Tape1 (or 2,3, or 4), so it cannot verify that it
loaded the tape into the drive it thought it did ... The only way I can
get the tape out of the drive is to manually open the library, push the
eject button on the drive, and then manually remove the tape and then
place it into one of the mail-slots...
