What you are seeing is an inconsistency in the media database - the
core problem is that when it first tried to recycle the tape there was
only a partial success of the operation - eg it deletes the record from
the database but fails to actually write a new label at the front of the
tape... Not sure why it failed to write to the tape, but the result was
that the media db then has different data to the tape and manual actions
are needed to tidy this up.

You have done quite a lot of testing already - but you should be
absolutely sure your hardware is working properly, there may be some
intermittent problem such as your drive is getting close to needing a
clean, the cleaning tape is expired, or the tape media you have is not
100% compatible with your drives (there is sometimes a case that LTO2
tapes from certain manufacturers does not work perfectly with certain
drives, see in this list's archives or ask the drive vendor if they know
of a problem with your media brand).

However - problems with drives and tapes should be picked up by
Networker and handled properly, but there are a few releases of
networker that have shortcomings in this bit of the process - you state
your version is 7.2. If you are actually using the original dot zero
release of 7.2 then you need to upgrade - that was a horrible release
with many bugs fixed in 7.2.1 and the various 7.2.2 versions. There were
in fact specific fixes in 7.2.2 build 494 to prevent failures to write
to a tape causing this mismatch of volids.

I would suggest you consider upgrading to at least 7.2.2 build 494, or
if possible 7.4.2 (note going to 7.3.4 or 7.4.2 means a siginificant
change to how the product is administered and works - you may want to
just try going to 7.2.2 to tackle the fault, and gear up for 7.4.2 as a
separate exercise, as there's no backing out from a 72>73+ upgrade).
