Just in case of
We had exactly the same problem with our LTO Tape Drive
After 3 months looking and searching at Software LEVEL, we called an IBM 
Engineer who simply, changed some configuration on the SCSI Card ( an 
adaptec ) and now, it's OK
Maybe, you can also send what are the configuration of your SCSI Card
