The drive will go into service mode after the   MAX CONSECUTIVE ERRORS
threshold is reached.

I run across this all the time if we load a bunch of new (unlabelled) tapes
and inventory them.  Each tape will generate two error messages.

We have also had it happen when we could not drive the tape drive fast
enough during a backup, or occasionally, a restore.  Usually cleaning the
drive 2 or 3 times then retrying would resolve this.

Check your /nsr/daemon.log and /nsr/messages files to see if
there are any device errors reported there.
