We run into this from time to time within our environment as well
(Win2K, NW 6.1.3).  We attempt to ensure this does not happen by making
sure that each pool has the appropriate number of blanks assigned to it
so NW doesn't have to think too hard.

However, we're only moving 1/2 a TB for our full backups, in a larger
environment, this would not be prudent.

Just a thought.

You're not missing the cause - it's a bug. I've seen this happen, and it
caused absolute havoc on a busy server - basically wrote off the backups
for the whole night. You've done the right thing to get things working
again, but there's nothing to stop it happening again. Raise a call with
Legato. Out of interest, what version are you running? I saw this with
6.1.1 on Solaris.
