The filesystem is OK; we used mkfile to create a 3GB file with no errors.

Drew,
It sounds like a large file problem with you OS. Can you crate a file which
is larger than 2 GB on your filesystem? If not, then I bet that scanner is
failing because the write failed. You can verify this by running scanner
through truss and seeing exactly what write failed. I am betting that it is
the write to the temporary file, nsrscan......
