[ADSM-L] Unexpected error 16 fetching row in table "Backup.Nameview"

Server is RH Linux 6.2.4.000 (yes, I am aware of the 6.2.4.200 patch but
nothing in the readme/descriptions address anything similar/related).
Client is W2K3 6.3.0.18 client
These errors are becoming a nightly problem. From the server side:

FWIW, this is our largest Windows server, object-wise. *>60M* objects
scanned. Total backups occupancy is >*378M* objects and 60TB. Running
Journal and MEMORYEFFICIENTBACKUP since the server only has 8GB memory.
Googling produces nothing usable/helpful. Looking for any ideas on how to
diagnose/resolve this.......
