Also, our entitlement on the PA site only allowed us to order 1 set of
cds even though we have 3 server instances in different geographical
locations.

We didn't have to order them before. They just arrived after couple of
weeks IBM published them to PA site.

You can order them from Passport advantage as well.
I went through the same thing yesterday and instead of being able to
order from the PA service center, they instructed me to use the PA
website.
