You can manually do the DSMFMT for a database and log file and then run
DSMSERV FORMAT to create the dsmserv.dsk file yourself. Or uninstall TSM,
expand /usr and re-install and it'll be done for you.

This is a new base install that just doesn't seem to have built this file
and no, I can't start the server to do anything because it can't find this
file.

The dsmserv.dsk is not "installed". It is created when you format the log
files using dsmserv format.... I recently upgraded our tsm services (3 of
them) from 4.2.2.3 to 5.1.6.2 on AIX and one of the files that remains
un-touched is the dsmserv.dsk file. I take it you are unable to start your
tsm service since the upgrade then, as if that file is missing, dsmserv
wouldn't know where to find the DB and recovery log volumes?
