The problem seems pretty obvious. A Netware service pack is added, and
then your backups become unreliable.

Assuming that you've done proper change control, check the versions of
TSA and SMDR that you're currently running vs. the versions that were
running prior to the service pack install. I'll bet you'll find a
change. If there was a change, see if you can backrev TSA and SMDR to
the levels they were prior to the upgrade.
