According to the TSM messages manual (RTFM, sir), ASC=4E and ASCQ=00
indicates 'overlapped commands attempt', which sounds to me like
something is getting overrun.

I'm not sure why IBM would replace the drive. Is it always the same one?
(We'll all assume you've done the simpler things like replace the fiber
cables/adapter.)

I'd kick this to Tivoli Support and let them wrassle with it.
