First move the data off the tapes to new one by doing a move data.
MOVE DATA VVVVVV
This will move the data to other tapes in the pool. If not all the data is
readable and it is a primary tape you can do a restore volume command if the
data is on a copy pool tape. If not, it is just gone and you just as well
do a delete volume vvvvvv discarddata=yes on the tape. The next backup will
re backup what was lost if it still resides on the client.
Checkout the tape immediately once it is scratch.

System: TSM 4.1.4 running on AIX 4.3.3
I have several tapes that TSM keeps switching the
ACCESS from 'READWRITE' to 'READONLY' because of media
fault. What do I need to do to replace these tapes?
Please advise.
