Has this configuration ever worked?

I assume you see the drives and library during the boot BIOS screens.  Duh,
of course since the library moves the tapes to the drives.

Have you crossed the device/element definition?  Back to the "has this ever
worked" question.  I'd suggest you use lbtest to look at this but lbtest
isn't shipping with the latest TSM server releases.  If you have an old
server lying around grab that code from the utils directory (along
acslib.dll) and use it: lbtest -dev lbx.0.0.y.

I poked around in the docs but could not find an error=121 so that remains a
mystery.

I guess in the realm of trying everything, replace the scsi card.
