the state would by Vault Retrieve, which means I should be able to check it
in.......

It failed because Access: Offsite and it is Empty. If using DRM do a:
 q drm 000254
to see what DRM state it is in. Then process it back through DRM states as you
would a returning tape from offsite. That will clear it to Scratch.

Ah:
Audit apparantly didn't fix it.

Now when I try to update the volume status I get:
tsm: BACKUP>upd vol 000254 acc=readw
ANR2032E UPDATE VOLUME: Command failed - internal server error detected.
ANS8001I Return code 4.

tsm: BACKUP>q vol 000254 f=d

Or get rid of it:
tsm: BACKUP>del vol 000254 discard=yes
ANR2221W This command will result in the deletion of all inventory references
to the data on volume 000254, thereby rendering the data unrecoverable.

Do you wish to proceed? (Yes/No) yes
ANS8001I Return code 14.

Larry - A frequently reported problem.
Try Audit Volume Fix=Yes, which usually takes care of it.
