I/O error on drive RMT1

From time to time I see errors like this. It looks to be a write error on a
tape. Sometimes the tape is put in a read only status sometimes not, in this
case it did. Can anyone give me an idea if this is just one of those things,
could it be dirty heads, anything? I only have two drives at the moment and
I've seen it on both.
