Somehow the volume got checked out of the library, or perhaps never got
checked in.  The way I would go about fixing this is as follows:

1. CHECKOUT LIBVOL <library> <volser> REMOVE=NO CHECKL=NO
2. Physically remove the tape from the library
3. CHECKIN LIBVOL <library> <volser>
