We had this last week:

ANR9999D mmsscsi.c(4284): Element address mismatch; slotInfo.elem = 33,
slotInv.addr = 32
ANR8832E CHECKIN LIBVOLUME for volumes in search mode in library STKR0
failed.

We ended up HALTing the ADSM server (3.1.2.1) and restarting it to clear
the problem.
