ANR9999D Segment not open for retrieval

This environment is NT4.0 SP3; ADSM is 3.1.2.0.
When the server is started, as an NT service or simply by
typing DSMSERV in the server directory, the error

ANR9999D dsrtrv.c (693) Segment not open for retrieval

is displayed.  This is followed by an access violation:

ANR9999D pkthread.c (2600) ADSM exception information:
Code=C00000005
Addr=1024E7F4
attempt to write data at address 4.

This system has been a little unstable for a week or so,
and today the system's owner found ADSM "dead" and each
attempt to restart gives this error.  The library was shut
off, thinking that perhaps library initialization was causing
the problem.  The system was completly powered off
and restarted, with the same problem.

Any ideas on how to resolve this problem?  Thanks.
