Lost VCR data

Hellp people,

I understand what VCR data is, but what I want to know is how can it get lost? 
is it simply read/write errors when updating the VCR data on a tape cause by 
dirty drives etc.. or is there some other cause? If Vital Cartridge Reords can 
be 'lost' from a tape, is there any cause for concern over normal data on the 
tape?

I ask because I am getting more 'Lost VCR' problems than I am read/write errors 
from the drives when using normal data.
