We are testing and have found the same results. We haven't gone live yet.
We backed up a server and tried to restore. We set the options to overwrite
open files and it kept failing then dumped the server with a kernel32
error.
The second step we did was build a recovery partition at least this time it
restores the open files however we had added 4 users to the registry the
accounts were missing after the restore.

This sounds to me as though you are not backing up the registry via the
REGBACK command. You should have a step in the Startup folder that does
this... Here's what to do:

1. Create a .BAT file in the directory where ADSM is - typically
WIN32APP/IBM/ADSM/BACLIENT. This BAT file contains one line as follows:

DSMC REGBACK USER CURUSER

Save this file as REGBACK.BAT (or similar)

2. Add a shortcut to the "Alluser" startup folder that points to the >BAT
file you just created.

When any user logs on, his registry info will be backed up now.

When you restore..... Follow the information in the IBM Redbook on Bare Metal
restores. The original book (I think a second one has been written) was for
NT3.51, but the steps for recovery were pretty close. The only major
difference was that when you were done with that procedure, you needed to do
a REGREST USER CURUSER command to get back the registry information
completely. After that was complete, Power off the machine (do not
shutdown), and reboot.

I have done this with NT Workstation V4, and had no problems.
