We have ADSM 2.1.5.13 - STK9710 at Code Level 60

We are experiencing the following messages and I cannot find the codes in the
appendix 'b'

ANR8300E I/O error on library xxxxx
(OP=00006C03,CC-304,KEY=04,ASC=40,ASCQ=02,Description=Changer Failure,
See Appendix B
