database errors causing transaction failures.

We are getting :
ANR0102E DSALLOC(881): Error 1 inserting row in table
"DS.Segments".
ANR0530W Transaction failed for session 889 - internal
server error detected.
and were wondering if anyone else has seen this and what was the 'fix'
We are on VM server .14 and tried .15 with no luck.
