If your operators do not tell Iron Mountain when to bring them back, or uses the "Infinite" date, IM just keeps them and continues to charge you for it.

Yes, we've seen this.  I'm not sure if there was a bug in NetBackup a while ago (or still there) or issues with our procedures.

Every now and then, we'll run a report from Iron Mountain looking for tapes with infinite or no expiration dates, recall them, and then revault to get a good date on them.
