Well, files are backed up sequentially across their blocks (first block in
the file to the last block in the file).  If that file's blocks are heavily
fragmented, you'll be wasting a lot of disk time seeking to data.  The
exception would be something like flashbackup that grabs the disk blocks
without much regard to the file structure.

How much of an issue can file fragmentation be as it relates to databases
and  backups? 

eg: 
Volume fragmentation 
    Total fragmentation                        = 47 % 
    File fragmentation                         = 95 % 
    Free space fragmentation                   = 0 %
