Justin,
 
I had similar sense key errors seen primarily when I was duplicating
tapes through Vault.  I had StorageTek upgrade the drive firmware to
K4B0/008.174, the errors are now gone.

After eight months of operation, I began to get these errors this
morning on one of two LTO2 SCSI drives.
Has anyone seen these errors before?
The backups are trying to write to the tape in this drive but it fails
and appears to only write to one drive.
I thought it was a bad tape but I replaced all of the tapes it had been
writing to with new fresh tapes this morning and the problem persists.
It looks as if the drive has gone bad?
Any ideas?
 
The OS is Solaris 10 on a V280R.
The tape library is a StorageTek L80 tape library with two
HP-Ultrium2-SCSI (LTO2) drives.
